Step 2 CS Blue sheet Case Hoarseness

These are the blue sheet, physical exam, diagnostic studies and differential diagnosis needed for a hoarseness case in the USMLE Step 2 CS exam:

BLUE SHEET HOARSENESS:

PHYSICAL EXAMINATION 

1. HEENT: 
Inspection conjunctiva, mouth and throat
Neck: thyroid exam
2. Lungs:  
Auscultation
3. Heart:
Auscultation
4. Abdomen:
Inspection
Auscultation
Palpation
Percussion
5. Extremities:
Inspection, tremors
6. Neurologic:
DTR’s for thyroid disease
            
POSSIBLE DIAGNOSTIC STUDIES:
 
  1. CBC.
  2. TSH, T4.
  3. ESR.
  4. U/S Neck.
  5. CT chest and abdomen (look for tumors).
  6. EKG/echocardiogram (mitral stenosis, atrial enlargement).
  7. Laryngoscopy.
  8. Esophageal PH monitoring (GERD).
 
DIFFERENTIAL DIAGNOSIS:
  1. Laryngitis (viral, voice overuse, etc).
  2. Laryngeal cancer.
  3. Laryngeal polyps or nodules.
  4. Hypothyroidism.
  5. GERD.
  6. Stroke.
  7. Goiter.
  8. Mitral valve stenosis.
